Referring a patient to Restore Fx is easier than you think! Our Administrator, Keisha Sims, will handle all of the pre-authorization for our evaluations and treatment. All you need to do is to contact her via email, phone or fax with the patient’s name, phone number, nature of the injury and any insurance information that you have.
Our Administrator will then prepare the necessary documentation to get evaluations approved and will schedule them. Your office will be kept in the loop at all times so that you know when your patient has been scheduled, what the results of any evaluations were and, if the patient is enrolled, how they are doing in the program on a weekly basis.
Please be aware that since Restore Fx functions as an interdisciplinary team, when you refer a patient for evaluation they will be seen by our Medical Director, Physical Therapist, Occupational Therapist and Behavioral Health Staff. This means they will undergo at least 4 different evaluations. We try to schedule all of these on the same day so that the patient can avoid numerous appointments and also to expedite the referral process. Once all of the evaluations are done copies will be sent to your office along with our recommendations.
Currently it takes on average 4-6 weeks from the time that you refer a patient to the time that they can be enrolled in actual treatment. And while insurance companies may have denied numerous types of services for your patient you can count on our program being approved. Since it’s inception in 2005 we have requested over 150 authorizations for care and have had only 3 of those denied. This is due to the overwhelming empirical support for functional restoration as well as the provisions within the actual Official Disability Guidelines that recommend this type of care.
