Case Management

The case manager is responsible for ensuring that the patient makes a smooth transition back to work within the first month post-discharge. He/she contacts employer or potential employers, insurance adjusters, nurse case managers, physicians and others involved in the treatment and return-to-work plan of the patient. The case manager works with the occupational therapist and patient to devise a return-to-work plan, including a “back-up” plan in the event that the patient cannot return to the previous position. The case manager is also responsibile for ensuring that the patients attend their 1, 2, 3 and 12 month follow-up visits and that all of the appropriate outcome data is collected and entered into the database for quarterly review.
